Special events held on City property (parades, street dances, walk-a-thons, rides/games, etc.) require a form to be submitted, approved, and insurance secured 30 days prior to an event.

Click here for a sample Certificate of Liability Insurance.  IMPORTANT:  Your certicate must be for $1,000,000 General Aggregate, the description must have the event and date listed, and the City must be listed as Additional Insured in the Certificate Holder box.
